Counselor Application Packet Application - Deadline: March 21, 2008
ELIGIBILITY
The Association of American Indian Physicians will select twelve counselors, age 21 and older, to attend the NNAYI program to be held in Washington D.C. Each counselor will serve as a role model and chaperone throughout the week for a select group of five to six high school students. American Indian/Alaska Native medical and health professional students preferred but not required. AAIP will cover travel, lodging, and most meals. Counselors will receive a stipend upon completion of the program. Counselors will be expected to arrive in Washington D.C. two days prior to program dates.
NOTICE: The Patty Iron Cloud National Native American Youth Initiative is NOT a summer camp - it is an intensive academic enrichment program that consists of a rigorous schedule of all-day sessions as well as continuous walking and/or walking long distances. You must be prepared to work hard and use your own initiative to take advantage of this learning opportunity.

APPLICATION PROCESS
____ Primary Data Sheet (located in application packet)
____ One-Page Personal Statement (double spaced) – Express why you are qualified/eager to work with AI/AN youth.
____ Most Recent Academic Transcript – Official transcript recommended for college students. Transcript not required for applicants not attending a college or professional school.
____ Resume – include work related experience with youth (both paid and volunteer)
____ Recommendation Checklist Form (located in application packet)
____ Letter of Recommendation (academic advisor, college instructor, or employer) – must be in a sealed envelope and attached to your application.
____ Photograph (for identification and publication purposes)
