The National Program to Eliminate Diabetes-Related Health Disparities in Vulnerable Populations
AAIP worked with three American Indian communities in medically under-served neighborhoods to develop system-based approaches to reduce risk factors associated with diabetes complications. The project was titled "National Program to Eliminate Diabetes-Related Health Disparities in Vulnerable Populations" and funded by the Centers for Disease Control and Prevention (CDC).
AAIP partnered with community-based coalitions to effectively plan, develop, implement, and evaluate evidence-based and culturally appropriate strategies.
